Homebase Medical is growing in the San Diego area - La Mesa/City Heights. We are currently searching for an experienced Bilingual Spanish Nurse Practitioners or Physician Assistants to engage in Medicare Wellness Visits for part-time hours for an 11-month contract at our contracted partner clinics.
**Target start date is Dec 2nd for training/onboarding and go live January 2**
**This is a part-time position of 3 days per week, 8-hour days (24 hours per week)**
**Must cover 3 days a week 8-hour days: Tuesday and Wednesday and 1 other day flexible depending on company need**
**This role will cover partner clinics for Medicare Annual Wellness Visits**
Responsibilities include:
- Perform Wellness Visits in partner primary care clinics and facilities in the La Mesa/City Heights community.
- Drive to designated facility locations in your assigned geographic area.
- Review patient medical and family history.
- Review of current providers and prescriptions.
- Perform height, weight, blood pressure, and other routine measurements.
- Deliver personalized health advice.
- Review risk factors and potential treatment options.
- Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
- Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
- Provide referrals to appropriate medical resources.
- Utilize our EMR system to document patient visit/interaction accurately and timely.
- Knowledge of HCC coding.
- Other duties as assigned.
Qualifications and experience:
- Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
- Current, unrestricted NP or PA license to practice in California, including DEA license.
- Minimum of 2 years of experience in primary care, hospice, or urgent care/outpatient setting.
- Minimum of 1 year of previous experience utilizing an EMR system.
- Good working knowledge of HCC coding.
- Strong computer skills including MS Office and Outlook is strongly desired.
- Great verbal and written communication skills
- Current drivers' license, auto insurance and reliable transportation
