Job Roles

  • Collaborate with multidisciplinary care team in coordinating a wide array of outreach, health care, case management, patient engagement functions in supporting and connecting patients of the Homeless Population with FHCSD services.
  • Develop relationships with this special population by use of best practices techniques involving Motivational interviewing, Trauma Informed Care, Critical Time Intervention, Harm Reduction, and Housing First modalities.
  • Provide direct and indirect patient services to support linkage and maintenance in care to include internal completion of internal and external referrals for the assigned programs.
  • Resolve any barriers related to necessary resources in supporting this special population.
  • As the intermediary, assist in communicating and clarifying information to access of needed resources.
  • Provide relative information on specific services and resources available that can be accessed and utilized.
  • Assist client in navigating through the various services in connecting to the most appropriate and viable resources.
  • Interact with all levels of the organization, in developing and sustaining relationships to service clients' needs and resources.
  • May be involved with patient quality improvement projects to include improving retention in connected resources and services.
  • Perform other duties as assigned.

Education/Certifications/Licenses/Registrations

  • GED equivalent or High School diploma required.
  • Bachelor's degree in social work, public health, science, or related degree preferred. Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.

Experience/Specialized skills (including Language)

  • 2 years of experience with health education or working in the medical/healthcare related field.
  • Knowledge of adult health coaching and engagement techniques, best practices and tools.
  • Ability to translate best practices into meaningful, culturally appropriate resources.
  • Ability to engage clients in meeting their healthcare needs to include cultivating patent connectedness with FHCSD services through care coordination and case management strategies.
  • Ability to exercise discretion, and maintain confidential information.
  • Ability to work in a fast-paced environment in seeking solutions and service connections for clients.
  • Ability to display cultural sensitivity and respect to ethnically and culturally diverse populations.
  • Knowledge of a wide array of services and resources related to patient engagement, healthcare, case management, and/or care coordination.
  • Knowledge of the community resources, health and social services systems in San Diego County, and possess skill to establish working relationships with community partners.
  • Possess critical thinking skills in finding solutions to problems/addressing barriers.
  • Strong interpersonal and customer service skills with the ability to interact empathetically with persons who possess diverse levels of socio-economic and education backgrounds.
  • Strong verbal and written communication skills to include the ability to communicate effectively with those who speak English as a second language.
  • Bilingual in English/Spanish preferred.
  • Basic computer literacy to comply with department needs and expectations.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

$19.00 - $23.11

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
